physiological or biological causes Theories under investigation There are many theories under investigation. What do they all have in common? They reiterate that endometriosis is a multifactorial disease - there are several factors involved in the disease - and that it creates vicious cycles in our system. A. Theory of retrograde menstruation When the uterus is unable to expel menstruation properly, menstruation with remnants of endometrium can rise and settle in the pelvis, causing endometriosis. However, this theory alone is not enough, since: There are men who have endometriosis [1] 90% of women have had or have retrograde periods and only around 10-15% develop endometriosis [2] It does not explain how endometriosis can appear so far from the pelvis, such as in the eyes [3] Does not explain deep infiltrative endometriosis [4] However, it remains a very important theory and with great probability the first trigger of this multifactorial disease, which will then develop or not and to what extent, depending on many other factors. In itself, the dysfunction of uterine contractions (“uterine dysperitalism”) also deserves an exhaustive analysis and is something that I incorporate in the elaboration of EndoYoga practices. B. Surgeries Another possibility for endometrial cells to end up outside the uterine cavity is through “contamination” during surgery in the abdominal area such as a cesarean section or hysterectomy. Imagine that endometrial cells “fall” out of the uterus during surgery and stay there. Like the previous theory, here too we need to explain what causes the body to not be able to “eliminate” these cells that are “aliens” outside the uterus. C. Genetics An article from February 2023 reveals that there are already 40 genetic risk factors mapped as important factors in the development of endometriosis and its extent. However, the dynamics between each of them require much more investigation. These are genes for hormonal regulation, cell adhesion and proliferation, particularly in stages 3 and 4 of the disease. We can say that a genetic risk profile for endometriosis for genes that cause mutations is beginning to emerge, something similar to carcinogens [2] . Once again, it is important to emphasize that endometriosis is benign. However, some health professionals call it “benign cancer” precisely because of its genetic mutations and because it can appear in areas far from its origin. Consequently, the most serious cases of endometriosis can actually pose very serious dangers to a woman's health. Epigenetics is addressed in environmental and behavioral factors. D. Metaplasia theories (cell mutation or transformation) There are several theories of metaplasia: Coelomic metaplasia: The development of female sexual organs in the embryo occurs from a pair of Müllerian ducts, which arise from coelomic epithelial cells of mesodermal origin. This theory assumes that the original coelomic membrane undergoes metaplasia and forms stroma and endometrial glands and is the most plausible theory for endometriosis in men, for example [5] . Metaplasia of embryonic Müllerian duct cells: Similar to the previous one, but here the mutation occurs “at the second level”, at the level of the Müllerian duct cells themselves [4] . Metaplasia of peritoneal cells: hormonal or immune system changes that can change the cells of the abdomen (peritoneal) into cells similar to those of the endometrium [4][5] . Mutation of embryonic stem cells: Either the endometrium or the bone marrow. The latter explains the appearance (although rare) of endometriosis outside the pelvis, such as in the eyes [3] . E. Sampson's theory of benign metastasis - Transport of cells via blood and/or lymphatic vessels According to this theory, endometrial cells can reach sites as remote as the brain by transport via blood and/or lymphatic vessels [4] . F. Hormonal imbalance and immune dysregulation The imbalance in estrogen levels also leads to a deficiency and resistance to progesterone. Progesterone would be the hormone that helps regulate estrogen. It is a harmful cycle that feeds on itself and contributes to the fact that 50% of women with endo suffer or will suffer from anxiety and/or depression [4] . After all, we are talking about the hormones that are the leading figures in the women's mood show. In turn, hormonal imbalance itself promotes conditions for inflammation. Menstrual cycles change and consequently the body's immune mechanisms fail due to unregulated levels of cells that would correct the situation. Not only menstruation is no longer expelled properly, but its remains are no longer properly eliminated as they would be under normal circumstances by the body, which leads to chronic inflammation. Other tumor-promoting and angiogenic factors support the progression of endometrial lesions and the inhibition of apoptosis (cells from unexpelled menstrual debris do not die as they should) promotes cell proliferation and lesion growth. In addition to the abnormal tissue growth promoted, there is the growth of nerve fibers, which can justify pain that can reach levels on the limit of the unbearable [4] . G. Bacterial infections This theory avenue focuses on the role of bacteria as the cause or progression of the disease. Chronic inflammation can also be explained by bacterial endotoxins and studies reveal that women with endo have deregulated microbiota (quantity and type of bacteria) in the intestine, vagina and uterus. They present higher levels of endotoxins in the menstrual and peritoneal flow. Namely, studies show that women with endo are more likely to have menstrual blood contaminated with the bacteria Escherichia coli . We also know that certain bacteria will inhibit and others will inhibit the progression of endo [2] . A June 2023 study found the bacteria Fusobacterium to play a determining role in the formation of endometriosis in the ovaries. This bacteria was found in 64% of patients with endometriosis. The researchers infected mice with Fusobacterium and implanted endometrial cells. They then tested the use of antibiotics and the results are promising: to a large extent they found that the implantation of endometriosis was reduced, as well as the number and severity of the lesions. Endometriosis When it hurts to be a woman What is endometriosis? Endometriosis, or “endo”, is a chronic inflammatory disease that affects mainly women of reproductive age. Its cause is yet unknown. It happens when tissue similar to the endometrium grows outside the uterus. The endometrium is the layer of the uterus that is shed monthly when a woman is not pregnant. It is therefore an estrogen-dependent disease. This tissue is often called endometriosis implants or endometriosis lesions. They are typically located in the pelvis, including the ovaries, Fallopian tubes and behind the uterus. This tissue behaves like the endometrium: it increases following menstruation and when there is no pregnancy, it bleeds. As there is no way for this blood to be expelled by the body, unlike the release of the endometrium vaginally during menstruation, it forms fibrous scar tissue. It also accumulates in blood cysts, also known as chocolate cysts due to their brown color from blood oxidation. This generates chronic inflammation in the affected areas, which is painful to the point of being disabling and which is associated with a series of symptoms and diseases depending on the areas impacted. 30% to 50% of people with endo will have infertility problems [1] Between 35% and 50% of infertile women are diagnosed with endometriosis [1] 71-87% of women with chronic pelvic pain have endometriosis [2] 50% will develop anxiety and / or depression [3] Most women have minimal or mild endometriosis, i.e. stages 1 and 2 [1] . However, that does not mean that they suffer any less than women in stages 3 and 4. Endometriosis impacts several areas and the overall life quality of a woman. Her career, sexual life, her social relationships, fertility, mood and humour, finances... When I began my research to collect and substantiate the endometriosis figures that I share here, I came across two major problems. The first is that, as incredible as it may seem, there are no consolidated statistics on this disease that affects so many millions of women! The second is that the numbers that exist do not add up and only reveal the extent to which we are dealing with an undervalued and little studied disease: It affects 10% of women of reproductive age , that is, 190 million adolescents and women worldwide, according to the World Health Organization, in an article published on its website in 2023 [4] . If we consider the same criteria as the Portuguese National Statistics Institute, women of childbearing age are considered to be those between 15 and 49 years old [5] . According to Pordata data updated on March 17, 2023, there were 2.223.648 women resident in Portugal in 2023. 10% would be 222.365 women in Portugal . However, curiously on the same day, Agência Lusa reported a survey conducted in 2022 by the Portuguese Society of Gynechology which pointed to "the existence in the country of around 350 thousand women with the pathology, one in 10 of childbearing age, the majority between 30 and 35 years old."[6] . Clearly, the math doesn't add up. If we consider the number of 350 thousand women, then we are not dealing with one in 10 women, but rather 15.74% of women aged 15 to 49 . If we consider that menarche in Portugal is getting closer and closer to happen at age 10, then we can include in the concept of "woman of childbearing age" females between 10 and 49 years of age, 2.247.475 people. 350 thousand would then be 15.57%. Still very different from 10%. Gynechologist Dr. Margarida Martinho, in an article published by Público in 2024, tells us that there are an estimated 228 thousand cases diagnosed in Portugal [7] Finally, if we take into account the article by gynechologist Dr. António Setúbal on the Hospital da Luz website, “it is estimated that the incidence of endometriosis in Portugal is around 700,000 cases”, then we are faced with the virtiginous number 31.48% of women of childbearing age. A third of us have endometriosis and over two thirds, 67.43% are not diagnosed [8] . To give a perspective of the magnitude, whatever number we consider, if you are a woman of childbearing age, it is much more likely that you have endometriosis than diabetes . endometriosis symptoms and related diseases Every woman is unique. So it is her endometriosis! It's not a cliché. The location, depth of the injuries and impact of the consequences vary immensely. It is a complex, dynamic and in many cases multidisciplinary disease. The possible symptoms are very diverse and can vary throughout the menstrual cycle . So whenever someone tells me they have endo, I ask where? How is YOUR endo? As a woman with endometriosis, you will have your own list of symptoms or even none at all for now! It doesn't predict at all what you feel or will feel. It's just an extensive reference that can be useful for anyone who wants to know more about endometriosis. Above all, it I compiled this list to create Hatha Yoga practices more adapted to the situation/situations we want to improve, that take into account the most diverse aspects of endometriosis. Sequences that adapt to you, your menstrual cycle and the symptoms you experience in each phase. I grouped the symptoms and related diseases into: Most common warning symptoms - the 4D‘s Depending on the location of the pain and the system impacted General As I mentioned in the stages and the myths about endometriosis, endo is not necessarily restricted to the female reproductive system. It can appear anywhere, even in the eye! It is important that we have this awareness so that the diagnosis is more timely and that endometriosis can be screened for even with less obvious symptoms. However, I don't want to cause alarm. The areas of the uterus, ovaries and tubes are clearly the most frequently affected. Very distant areas such as the eyes, for example, are a very rare exception [1] . 1. Most common warning symptoms - the 4D’s According to Mulherendo, a Portuguese association supporting women with endometriosis, there are 4 D's that we should pay attention to: Dysmenorrhea - pain during menstruation Dyschezia - pain when defecating Dyspareunia - pain during sexual intercourse Dysuria - pain when urinating Endometriosis Arrow right Endo stages Arrow right Causes Arrow right Myths Arrow right 2. Symptoms and related illnesses depending on the location of the pain and the system impacted Gynecological: Symptoms Dysmenorrhea: menstrual pain that can be disabling and that does not subside with common anti-inflammatories Dyspareunia: pain during sexual intercourse Heavy menstruation Irregular cycles Spotting, small flow losses and brown fabric Abdominal pain Low back pain Related diseases Infertility Adenomyosis, endometriosis of the uterus. It occurs when endometrial cells infiltrate the myometrium, causing the walls of the uterus to thicken and painful and heavy periods. Anemia resulting from heavy or too frequent periods Sexual dysfunction Gastrointestinal: Symptoms Dyschezia: pain when evacuating Intense intestinal cramps Blood loss in stool Nausea and vomiting Diarrhea and/or constipation that can alternate Abdominal distension, the famous “Endo Belly” Flatulence Abdominal pain Related diseases Irritable Bowel Syndrome Urological: Symptoms Dysuria: Painful urination Frequent urge to urinate Weight in the bladder area Abdominal pain Low back pain Blood loss in urine during menstruation Related diseases Cystitis or irritable bladder Recurrent urinary infections, without necessarily the presence of bacteria Kidney failure in extreme cases, for example because the ureters are blocked by adhesions Others. As there is no limit to where endometriosis can appear, consequently the symptoms will be very diverse. For example, endometriosis in the diaphragm causes difficulty and pain when breathing. 3. Quando comecei a minha investigação para fundamentar os números da endometriose deparei-me com dois grandes problemas. O primeiro é que por incrível que pareça, não há estatísticas consolidadas sobre esta doença que afeta tantos milhões de mulheres! O segundo, é que os números que há não batem certo e apenas revelam o quanto estamos perante uma doença subvalorizada e pouco estudada: Afeta 10% das mulheres em idade reprodutiva , ou seja, 190 milhões de adolescentes e mulheres em todo o mundo, de acordo com a Organização Mundial de Saúde, num artigo publicado no seu website em 2023 [1] . Se considerarmos o mesmo critério que o Instituto Nacional de Estatística, considera-se mulher em idade fértil a que está entre os 15 e os 49 anos [2] . De acordo com os dados da Pordata atualizados a 17 de março de 2023, havia 2.223.648 mulheres residentes em Portugal em 2023. 10% seriam 222.365 mulheres em Portugal. Porém, curiosamente no mesmíssimo dia, é noticiado pela Agência Lusa um inquérito conduzido em 2022 pela Sociedade Portuguesa de Ginecologia que apontava para "a existência no país de cerca de 350 mil mulheres com a patologia, uma em cada 10 em idade fértil , a maioria entre os 30 e os 35 anos." [3] . Claramente, a matemática não bate certo. Se considerarmos o número de 350 mil mulheres, então não estamos perante uma cada 10 mulheres, mas sim 15.74% das mulheres dos 15 aos 49 anos . A ginecologista Dra. Margarida Martinho, num artigo publicado pelo Público em 2024, dá-nos conta que estima-se que hajam 228 mil casos diagnosticados em Portugal [4] . Finalmente, se tivermos em linha de conta o artigo do ginecologista Dr. António Setúbal no site do Hospital da Luz que nos relata que "estima-se que a incidência de endometriose em Portugal seja de cerca de 700 000 casos", ent ão estamos perante o virtiginoso número 31.48% das mulheres em idade fértil. Um terço de nós tem endometriose e mais de dois terços, 67.43% não está diagnosticada [5] . A título de curiosidade e para dar aqui uma perspetiva da ordem de grandeza, qualquer que seja o número que considerarmos, se és mulher em idade fértil, é muito mais provável teres endometriose do que diabetes . Em 2018, a diabetes afetava apenas 1.4% das mulheres entre os 20 e os 39 anos [6]. Perante estatísticas tão amargas, o melhor mesmo é evitar os doces. FALTA AQUI % que tem dor pélvica e dados infertilidade Dor menstrual não é normal. Dor a fazer amor não é frescor. 7 anos e 7 a 8 médicos depois... Os obstáculos ao diagnóstico de endometriose Ser mulher é dor "H á um atraso de cerca de sete anos no diagnóstico de endometriose. A mulher passa entre sete a oito médicos até chegar ao diagnóstico” - quem o diz é o Presidente da Sociedade Portuguesa de Medicina de Reprodução (SPMR) para a Região Sul em entrevista à CNN Portugal []. Em causa está antes de tudo uma cultura da dor na mulher . Somos criadas para aceitar passivamente que tenhamos de sofrer na menstruação e às vezes até no sexo. "A tua avó também sofria muito!", "A filha da vizinha outro dia quase desmaiou!", "Mulher sofre, minha filha..." e por aí vai. Por todo o lado crescemos rodeadas de como ser mulher é uma sentença à dor a vida inteira por todos os motivos e mais alguns. Por isso, muitas mulheres suportam e calam as dores, acreditando que é normal. Dor menstrual não é normal. Dor a fazer amor não é frescor. Se a cólica menstrual não cede a um paracetamol, algo não está bem. Se há mais que um ligeiro desarranjo intestinal, algo não está bem. Se o sexo dói, algo não está bem. Descriminação Qual é a mulher que nunca se sentiu descriminada, desrespeitada e silenciada baixo o preconceito de histérica, neurótica, exagerada e, mais recentemente, como alguém que cujos sintomas são psicossomáticos? Há apenas 100 atrás, Freud e o seu famoso e prodigioso discípulo Reich consagravam na psicanálise a ostracização da mulher como ser psicologicamente débil e dado a achaques nervosos. Tanto assim que se chegou ao ponto de induzir orgasmos às pacientes como tratamento. Ou choques elétricos... Não é de estranhar que nós mulheres nos calemos. E que as que não se calem fiquem sem resposta às suas queixas ou oiçam algum comentário desagradável. Dificuldade de diagnóstico Os tradicionais exames anuais de prevenção cingem-se a ecografias endovaginais e análises ao sangue. As análises ao sangue não acusam endometriose. E a eficácia das ecografias endovaginais vai depender muito do padrão da doença na doente e da perícia do técnico. As células endometriais podem ser um pontilhado disperso difícil de observar e, logo, de diagnosticar. A isto acresce a enorme variedade de localizações da endometriose e possíveis sintomas. Imaginemos que o seu maior sintoma é uma dor no glúteo. A sua endometriose pode estar a afetar o nervo ciático. Tratar a endometriose Os tratamentos para a endometriose dependem da gravidade e do objetivo. A primeira linha de tratamento é a terapia hormonal para reduzir dores e fluxo menstrual ou até mesmo inibir a menstruação. Paralelamente com a analgésicos mais fortes, ou combinado com um anti-espasmódico. A fisioterapia pélvica pode também ser importante para gerir a dor crónica ou reduzir as cicatrizes de endometriose ou da cirurgia, nomeadamente com radio frequência. Pode ser necessário ou aconselhável a criopreservação de óvulos ou fertilizações in vitro . Nos casos mais severos, em que hajam muitas queixas, perigo ou mesmo em que órgãos estão a ser impactados, a cirurgia pode ser necessária. Esta pode ser mais ou menos conservadora, consoante o caso. Tenta-se sempre evitar a remoção dos ovários e do útero, o que causaria todas as complicações de uma menopausa precoce. endo stages American Society of Reproductive Medicine (ASRM) classification Endometriosis Arrow right Symptoms and related diseases Arrow right Causes Arrow right Myths Arrow right There is no consensus on the classification of endometriosis. The most used system is that of the American Society of Reproductive Medicine (ASRM). This system was designed with a focus on the impact on fertility and not necessarily the impact on patients' quality of life . Endometriosis is classified into four stages depending on the location, extent and depth of endometriosis implants and adhesions. Stage 1, minimal endometriosis (5 points or less), also called superficial peritoneal endometriosis The lesions are small and superficial. Scar tissue is minimal or non-existent. Stage 2, mild endometriosis (between 5 and 15 points), endometriomas More injuries and and they tend to be located deeper inside the tissue. There may be some scar tissue, but there is usually no inflammation yet. Stage 3, moderate endometriosis (between 16 and 40 points), also known as deep infiltrative endometriosis Presence of multiple and deep endometriosis implants. There may be endometriomas or chocolate cysts on the ovaries. These cysts are formed when implants surround the ovaries and when they bleed, they leave old brown blood that collects in a capsule that can grow and be very painful when they leak. These bands of scar tissue can fuse the organs together. Stage 4, severe endometriosis (more than 40 points), or abdominal wall endometriosis Infertility at this stage is very likely due to distortion of the morphology of the abdominal wall. It is characterised by numerous and deep implants. The implants can be in the fallopian tubes and intestines. Endometriosis adhesions can be thick and deep and cause severe pain. There may be numerous and/or large cysts on the ovaries, and between the uterus and rectum. Enzian survey Another classification, more for the purposes of evaluating deep endometriosis , is the Enzian. The questionnaire goes far beyond the typical areas of endometriosis: ovaries, tubes and peritoneum, the membrane that lines the abdominopelvic cavity and covers its organs. Includes scoring for injuries in the following locations: Vagina Rectovaginal space, area between the vagina and rectum, the final part of the large intestine Retrocervical area, i.e. behind the cervix Pelvic wall Cardinal ligaments that connect the uterus to the pelvis Sacral uterine ligaments that connect the uterus to the sacrum bone) Pelvic sidewall Adenomyosis Bladder Intestines Ureters Other locations: diaphragm, lungs, nerves… Endometriosis can appear anywhere. endo myths Endo myths Below I list some of the most common and, I believe, some of the most disconcerting myths about endometriosis, all of which are false. It affects only women – FALSE It only affects women over 20 years old - FALSE It only affects women who have never given birth - FALSE It disappears with pregnancy - FALSE It only affects the female reproductive system - FALSE It disappears if we remove the uterus - FALSE It disappears during menopause - FALSE The level of pain corresponds to the stage of the disease - FALSE 1. It only affects women FALSE. Although extremely rare, there are cases of men who have developed endometriosis. Particularly, men who have taken hormone therapy with oestrogens to treat prostatic cancer and men with liver cirrhosis [1] . 2. Only affects women over 20 FALSE. Teenagers also have endometriosis! Endometriosis is the main cause of dysmenorrhea (menstrual pain) in adolescents. In fact, studies indicate that endometriosis can form even before menarche and that 66% of adult women with endo report having had symptoms before the age of 20 [2] . 3. It only affects women who have never given birth FALSE. This is a prejudice that only began to dissolve in the mid-1990s! For many years, doctors and researchers postulated that endo affected “career women,” i.e., those who delayed motherhood [3] . 4. Disappears with pregnancy FALSE. Pregnancy decreases oestrogen production. Therefore, during pregnancy, the condition improves. However, it returns with the normalisation of hormone production. On the other hand, “complications during pregnancy are more frequent in women with endometriosis: premature birth, placenta previa, babies with lower weight at birth”… [4] . In most cases, patients are mainly affected in the female reproductive organs, particularly in stage 1 and 2 endometriosis. However, endometriosis can appear anywhere, even in the eyes [4] . 6. It disappears if we remove the uterus FALSE . If you do not remove all pre-existing endometriosis, which may be impossible (imagine a dots spread over an area), the ovaries will continue to produce oestrogen and therefore feeding the endometriosis [4] . 7. It disappears during menopause FALSE. It is true that during menopause the ovaries no longer produce the same amount of oestrogen, the tendency is for endometriosis to regress and no longer cause symptoms. However, 2%–4% of menopausal women experience symptoms of endometriosis [5] . One must also consider that many menopausal women need to take hormone replacement therapy with oestrogen [6]. 8. the level of pain corresponds to the stage of the disease FALSE. Most of the time, the scale of the disease refers to purely physiological factors and not exactly the level of pain (see stages of the disease ). Women with endometriosis only in the ovaries can suffer terrible pain during menstruation, while other women can have endometriosis already infiltrated in the intestines and still have no symptoms or very mild symptoms that don't even worry them.
